Schneck Medical Center is expanding its reach and services in the community by forming a new primary health care practice.

The Seymour hospital announced Monday it will take over operations of Jackson Park Family Physicians and Urgent Care of Seymour.

Beginning Saturday, the practice will do business as Schneck Primary Care and will include both the Seymour office at 1124 Medical Place off South Jackson Park Drive, Brownstown Family Care at 806 W. Commerce St. and the renamed Schneck Urgent Care, located at 1130 Medical Place.

The merger is a natural extension of Jackson Park Family Physicians’ long relationship with the hospital and will enable the practice to serve its patients with an even more seamless and well-coordinated level of comprehensive care, hospital officials said.
